To move to a month-to-month contract at the end of your current term, please fill out the form attached to this page and return to sales@blazeportfolio.com.

Please review your Blaze Portfolio contract to familiarize yourself with your renewal date or contact sales@blazeportfolio.com to confirm the renewal date we have on file.

Month-to-month extensions will require sign-up on our 3rd party auto-pay platform AdvicePay, please ensure you include a preferred billing contact with your completed form for setup on the AdvicePay platform (Credit Card & ACH payments accepted).
